The ideal candidate should have a strong background in home health care and possess exceptional interpersonal and counseling skills to support patients and their families in navigating the challenges associated with health issues. Responsibilities: Conduct comprehensive psychosocial assessments of patients and their families, identifying emotional, social, and environmental factors impacting the patient's well-being and care. Collaborate with interdisciplinary team members, including nurses, therapists, and other healthcare professionals, to develop and implement holistic care plans addressing the psychosocial needs of patients. Assist patients in accessing community resources, support services, and financial assistance programs to enhance their overall quality of life. Advocate for patients and families in navigating the healthcare system, addressing barriers to care and facilitating communication between healthcare providers. Participate in case conferences and team meetings to discuss patient care plans and contribute to overall care coordination. Document patient assessments, interventions, and follow-up plans accurately and timely in accordance with regulatory standards. Collaborate with community agencies and organizations to strengthen the network of support available to patients outside the clinical setting. Stay informed about changes in healthcare policies, social work practices, and home health regulations to provide up-to-date and effective support. Adhere to ethical and professional standards, ensuring patient confidentiality and fostering a supportive and caring environment. Qualifications: A Master's or Doctorate degree in Social Work from an accredited program accredited by the Council on Social Work Education. Minimum of 1 year of experience in home health social work and/or in an acute care setting. Strong clinical assessment and counseling skills with a focus on patient-centered care. Excellent communication and interpersonal skills. Ability to work independently and collaboratively within a team. Understanding of Medicare home health regulations and documentation requirements.
