Case Manager - Full-Time - Saint Louis Overview: The Registered
Nurse - Home Health Case Manager delivers expert nursing care to
patients in their residences, working closely with physicians to
evaluate, plan, execute, and document care. This role demands
strong clinical assessment abilities, effective communication, and
the capacity to handle a varied caseload while aiding in patient
recovery and managing chronic illnesses. This full-time position

you will be responsible for delivering skilled nursing services to
patients in their homes under physician supervision. Your duties
will include assessing, planning, implementing, documenting,
evaluating, and educating patients about their care needs. Position
Details: Home Health RN (Community-Based) Opportunities Available
in the St. Louis Metro Area and Surrounding Counties Full-Time (40
hours per week), Day Shift Monday to Friday, 8 AM - 4:30 PM on-call
weekends Holiday Rotation - 1 summer, 1 winter Gas Mileage
Reimbursement - $0.70/mile Subject to adjustments based on Federal

Qualifications: Experience: Preferably one year in a related field.
Education: Must have graduated from an accredited nursing program.
Licensure: Current RN license in Missouri. Must possess a valid and
insured driver's license. Certifications: BLS CPR Certification
from the American Heart Association. Additional: A home health
nurse manages a diverse caseload of patients with various health
conditions. Responsibilities include providing essential patient
education, treatment, and emotional support to aid recovery from
injuries and illnesses, as well as managing chronic diseases. The
role requires excellent assessment skills and technical
proficiency. The ability to anticipate and respond to changes in
patients' clinical status is crucial. Building rapport and
effective communication with patients and their families is
essential. The home health nurse must maintain consistent and
professional communication with physicians and other healthcare
professionals regarding patient care, ensuring accurate
